Antonio Toesca, direttore della Chirurgia senologica dell’IRCCS di Candiolo e coordinatore dello studio, ha evidenziato che la mastectomia endoscopica è stata progettata per superare le limitazioni delle tecniche tradizionali, che spesso portano a cicatrici visibili e complicanze come la necrosi del complesso areola-capezzolo. La nuova tecnica, spostando l’incisione in aree meno esposte, consente di preservare il complesso areola-capezzolo e ridurre i rischi di complicanze post-operatorie.
Risultati dello studio: una riduzione significativa del dolore e delle complicanze
L’analisi dei dati ha rivelato che la mastectomia endoscopica è associata a una diminuzione del 20% del dolore post-operatorio e delle complicanze, in particolare della necrosi del complesso areola-capezzolo, rispetto alla tecnica convenzionale. Inoltre, la maggior parte delle pazienti ha potuto beneficiare di una ricostruzione immediata della mammella, con tassi di successo paragonabili a quelli della chirurgia tradizionale.
Toesca ha specificato che l’intervento eseguito tramite un’unica incisione nella regione ascellare ha dimostrato di rispettare maggiormente l’anatomia e la sensibilità cutanea, portando a un miglioramento della qualità della vita delle pazienti. L’unico aspetto negativo riscontrato è stato un tempo operatorio leggermente più lungo, ma il beneficio complessivo giustifica questo svantaggio.
Sicurezza oncologica e futuro della mastectomia endoscopica
Il gruppo di ricerca ha sottolineato che la sicurezza oncologica della mastectomia endoscopica è comparabile a quella della chirurgia tradizionale. I tassi di recidiva e il controllo locale della malattia sono risultati simili, ma con il vantaggio di un approccio meno invasivo sia fisicamente che psicologicamente.
Toesca ha concluso che la mastectomia endoscopica rappresenta una valida opzione per pazienti selezionate, a condizione che venga eseguita in centri con esperienza specifica e all’interno di un percorso multidisciplinare. Saranno necessari ulteriori studi con follow-up a lungo termine e protocolli standardizzati per confermare i risultati e valutare l’impatto duraturo sulla sopravvivenza e sulla qualità della vita. Salvatore Nieddu, direttore generale dell’IRCCS di Candiolo, ha affermato che questo studio evidenzia l’impegno dell’istituto nella ricerca clinica avanzata, orientata verso una chirurgia oncologica sempre più personalizzata e attenta al benessere delle pazienti.
